Your First Appointment

Please be on time and bring your membership number with you if you have medical insurance.

You will be asked to fill in a patient registration form if you have not already completed our online form.

Jason will review your case and discuss the options for your treatment.

While you are at your consultation appointment, an appointment for your surgery will be made.

Jason will also give you a quote for expected costs. You can use this quote to apply for prior approval with your insurance company.
